📝 Soungui Fitness Gym Access Membership Waiver

Waiver and Release of Liability By checking the box and submitting this form, I acknowledge and agree to the following terms as a condition of my membership and access to Soungui Fitness: 1. Assumption of Risk I understand that participation in physical activities and use of gym equipment involves inherent risks, including but not limited to: muscle strains, sprains, falls, cardiovascular events, and other potential injuries. I voluntarily accept full responsibility for any risks or injuries that may occur while using Soungui Fitness facilities. 2. Health Acknowledgment I confirm that I am physically capable of participating in fitness activities and have either consulted with a medical professional or chosen to participate at my own risk. I agree to notify Soungui Fitness of any medical conditions or limitations that may affect my participation. 3. Release of Liability I hereby release, waive, and discharge Soungui Fitness, its owners, employees, trainers, agents, and representatives from any and all liability, claims, demands, actions, or causes of action related to any loss, damage, or injury that may be sustained while participating in gym activities or while on the premises. 4. Code of Conduct I agree to abide by all posted and verbal rules of conduct, including respecting other members, using equipment properly, maintaining hygiene, and refraining from engaging in any dangerous or inappropriate behavior. 5. Photography/Video Consent I consent to the use of photos or video taken of me during workouts or on the premises for promotional and social media purposes by Soungui Fitness, unless I notify in writing to opt out. 6. Acknowledgment of Terms I have read and fully understand this waiver and voluntarily agree to its terms. By signing up for membership and accessing the gym, I confirm that this agreement is binding and applies to my participation in all present and future activities at Soungui Fitness.
